Hospital programs spotlighted by OCHNA include:
Mission Hospital
Stone Field Community Soccer Field: Community
Outreach at Mission Hospital has created a project
that stems from the community health needs assessment done
by the hospital
with other community partners that clearly showed that residents
felt parks and recreation were lacking in their neighborhood.
In response, Mission initiated a partnership with the City
of San Juan Capistrano to refurbish an historic site to
provide
a safe soccer facility for local residents. The cost of the
project was paid by funds from the St. Joseph Health System
Foundation and the City of San Juan Capistrano. The restored
field, which derives its name from the Works Progress Administration
(WPA)-era stone wall, which encloses it, opened in June
2002.
The field sits in the midst of a crowded residential area.
Over 400 children live in just one of three apartment complexes
in the neighborhood. A total of 24 youth teams use the
facility.
Members of the community work together to ensure that everyone
has access to the field and organize a community walking
patrol
to maintain a safe environment.
St. Jude Medical Center
Adult Mobile Health Clinic: The
Adult Mobile Health Clinic program at St. Jude Medical Center was developed in
1999 and merged the following year with the Healthy Beginnings
Prenatal Program. The mobile van visits different locations
in northern Orange County six days per week (Monday through
Saturday), receiving approximately 3,600 visits per year for
adult medical and prenatal services. The program is supported
by the hospital Care for the Underserved program, as well
as support coming from third party billing, grants, and donations.
On a typical day, volunteering physicians and nurse practitioners,
in each van will see 25 or 30 people. In addition to the adult
van, St. Jude Medical Center also sends out a children’s
van and a dental van to the same locations. For more information,

St. Joseph Hospital
Diabetes Initiative at La Amistad De Jose Clinic:
The
Diabetes Initiative at St. Joseph Hospital in Orange provides
a full spectrum of care for patients dealing with diabetes.
The program services are provided at the La Amistad de Jose
Clinic and gives patients the education and tools needed to
properly manage their health. The clinic opened in 1985 and
sees approximately 25 diabetes patients a day. The physicians
make the diagnosis at the clinic and then start the patient
on a program by giving them a home glucose meter and enrolling
them in education classes. A pharmacy system allows them to
get drugs from the drug companies at a significant discount.
The initiative also includes 17 or 18 other community clinics
that are working together to identify specialist care and
learn from one another’s practices. For appointment

Chapman Medical Center
Premier Advantage Program for Seniors: The primary
goal of Premier
Advantage is to promote healthy living. The
program offers a wide variety of lectures and seminars. It
includes free access to health education seminars, health
fairs and screenings. Members learn about the many innovative
services available to them. The program also provides valuable
discounts on a variety of products, services and benefits
from participating vendors. To qualify for Premier Advantage
for seniors, you need to be 55 years of age or older. Membership

CHOC
Healthy Tomorrows Mobile Clinic: The
Healthy Tomorrows program consists of four mobile clinics
that are equipped to provide health care services and wellness
education to
underserved children in Orange County. The mobile clinics
currently provide services to 35 elementary schools throughout
central and north Orange County. The mobile clinics are
staffed
with a Pediatric Nurse Practitioner, a nurse and a patient
services representative who provide school physicals, well-child
care, immunizations and sick-child care. For more information

Coastal Communities
Hospital
Growing Expectations OB Program: The
Growing Expectations Program at Coastal Communities Hospital provides an array
of services to pregnant women. They refer you to physicians
who provide free pregnancy testing and provide application
assistance for Medi-Cal and/or Healthy Families. They provide
local transportation to your doctor and hospital appointments
at no charge. In partnership with the Maternal Outreach Management
System (MOMS), Coastal Communities Hospital offers free prenatal
and parenting classes. These classes are taught in both English
and Spanish and childcare is provided for free. Another service
provided by the Growing Expectations program is Maternity
Tea where they provide a guided tour of the labor and delivery
ward, maternity floor and newborn nursery. Participants in
the program are sent home with a free infant car seat or
bassinet
at the time of delivery, as well as diapers. For more information

Hospital and Medical Center
Senior VIP Club: Fountain Valley Regional Hospital
and Medical Center developed the
Senior VIP club as a free
community service program for people 55 years and older. It
is designed to help Seniors enhance their current healthcare
coverage and stay current on some of the latest trends in
healthcare. Membership benefits include: discount pharmacy
program, free wellness & preventive medicine program,
free preventive medicine screenings, discounted health services,
emergency medical card, local non-emergency transportation
services, member services hotline, local merchant discounts,

Health Education Classes: Garden Grove Hospital
and Medical Center offers a wide range of healthy
education
programs to the community from prenatal care to general healthy
living. Many classes are free and are offered in both English
and Spanish. Classes offered include: Prepared Childbirth,
Breast Feeding/Infant Care, Infant CPR, Stress Management,
Nutrition
101, Diabetes Series, Cholesterol Hypertension and Heart
Disease,
