Hospital
Stone Field
Community Soccer Field: Community Outreach at Mission
Hospital has created a project that stems from the community
health needs assessment done by the hospital with other community
partners that clearly showed that residents felt parks and
recreation were lacking in their neighborhood. In response,
Mission initiated a partnership with the City of San Juan
Capistrano to refurbish an historic site to provide a safe
soccer facility for local residents. The cost of the project
was paid by funds from the St. Joseph Health System Foundation
and the City of San Juan Capistrano. The restored field, which
derives its name from the Works Progress Administration (WPA)-era
stone wall, which encloses it, opened in June 2002. The field
sits in the midst of a crowded residential area. Over 400
children live in just one of three apartment complexes in
the neighborhood. A total of 24 youth teams use the facility.
Members of the community work together to ensure that everyone
has access to the field and organize a community walking patrol
to maintain a safe environment.

Jude Medical Center
Community
Clinics.
St.
Jude Medical Center has established community clinics to serve
North Orange County uninsured and underinsured residents.
The goals of the clinics are to increase access to medical
and dental care for the underserved, to prevent unnecessary
emergency room admissions, to improve the health status of
underserved communities and to reduce health care disparities
in selected medical conditions. The Medical Center operates
the St. Jude Neighborhood Health Center in Fullerton , its
satellite in La Habra in partnership with the Institute for
Health Advancement, the St. Jude Mobile Family Health Clinic
serving sites in Placentia and Brea and the St. Jude Children’s
Dental Clinic in Buena Park in partnership with the Boys and
Girls Club of Buena Park. For more information, please

Obesity
Prevention Plan: St.
Jude Medical Center has implemented an Obesity Prevention
Plan to address the epidemic of childhood obesity. The
following programs have been implemented:

Fullerton
Healthy Neighborhood Project: California Endowment
funded partnership with the Fullerton Collaborative, OCCCO,
Solidarity and CSUF designed to work with four low income
neighborhoods in Fullerton to organize residents to address
policy issues impacting childhood obesity.

Healthy
for Life: Partnership with American Academy of Pediatrics,
Mission Hospital, St. Joseph Hospital, the St. Joseph
Health System Foundation and UCI implementing enhanced
physical education and nutrition programs in schools serving
low income populations. Over three years 100 additional
schools will participate in this program. Implementation
of the School Health Index is also part of this project.

Fit
by Design: Partnership with St. Jude Heritage
Medical Group and the YMCA of Orange County provides an
evidenced-base obesity protocol for the pediatric offices
and a 12 week outpatient group for overweight children
and their families.

Maternal
Depression Program: The Caring for Women with Maternal
Depression Program is part of the comprehensive services and
support that St. Joseph Hospital provides to their patients
and the larger community. In fact, 100 percent of
mothers who deliver at St. Joseph Hospital are screened for
postpartum depression through the Bridges for Newborns Program.
We also work in collaboration with a program called MOMS (Maternal
Outreach Management System) to provide services for our Spanish-speaking
patients.
Mother-Baby
Assessment Center: St.
Joseph Hospital 's award winning Mother-Baby Assessment Center
provides a safety net for new moms during that critical time
between hospital discharge and the first visit with a
physician.
Your first visit to the St. Joseph Hospital Mother-Baby Assessment
Center should occur when you find out you are pregnant. During
your visit, you will be given your "Pathway through
Pregnancy" booklet — your personal guide through your
maternity experience and your partnership with St. Joseph
Hospital . You and your baby will be scheduled for your Mother-Baby
Assessment Center follow-up appointment within one week after
delivery. The appointment will be made before your discharge
from the hospital. Our staff will work in conjunction with
your obstetrician and pediatrician to coordinate a healthy
transition for you and your baby.

Bridges
for Newborns: St. Joseph Hospital is participating in
Bridges for Newborns, one of four Early Action programs made
possible by a grant from the Orange County Children and Families
Commission. This grant aims to improve the health and welfare
of children in Orange County .
The
Bridges for Newborns program helps families link to a variety
of family-focused assistance programs, including help in identifying
important health insurance coverage options for your newborn.
Our goal is to provide access to quality medical care for
your baby, and help you give him or her a healthy start on
life.
CHOC
Healthy Tomorrows
Mobile Clinic: The
Healthy Tomorrows program consists of four mobile clinics
that are equipped to provide health care services and wellness
education to underserved children in Orange County. The mobile
clinics currently provide services to 35 elementary schools
throughout central and north Orange County. The mobile clinics
are staffed with a Pediatric Nurse Practitioner, a nurse and
a patient services representative who provide school physicals,
well-child care, immunizations and sick-child care. For more

Coastal
Communities Hospital
Growing Expectations
OB Program: The Growing Expectations Program at Coastal
Communities Hospital provides an array of services to pregnant
women. They refer you to physicians who provide free pregnancy
testing and provide application assistance for Medi-Cal and/or
Healthy Families. They provide local transportation to your
doctor and hospital appointments at no charge. In partnership
with the Maternal Outreach Management System (MOMS), Coastal
Communities Hospital offers free prenatal and parenting classes.
These classes are taught in both English and Spanish and childcare
is provided for free. Another service provided by the Growing
Expectations program is Maternity Tea where they provide a
guided tour of the labor and delivery ward, maternity floor
and newborn nursery. Participants in the program are sent
home with a free infant car seat or bassinet at the time of
delivery, as well as diapers. For more information about the

Valley Regional Hospital and Medical Center
Senior VIP Club:
Fountain Valley Regional Hospital and Medical Center developed
the Senior VIP club as a free community service program for
people 55 years and older. It is designed to help Seniors
enhance their current healthcare coverage and stay current
on some of the latest trends in healthcare. Membership benefits
include: discount pharmacy program, free wellness & preventive
medicine program, free preventive medicine screenings, discounted
health services, emergency medical card, local non-emergency
